Immunization Form

The Immunization Form requirement excludes students who are in degree programs that will only meet online like M.Ed. in Educational Administration/Adult and Higher Ed, M.Ed. in Technology, M.Ed. in Technology & Customized Learning, and Ed.D. Doctorate in Leadership. Those degree program students who have a combination of course delivery formats must submit immunization information.

Exemption form measles, mumps, rubella (MMR) vaccination (A student must legibly check the applicable box on the form):

• Birth Exemption (born prior to Jan 1, 1957
• Taking online course only
• Taking professional Development Workshop
• Have attended SD university prior to July 1, 2008
• Medical Exemption
• Religious Exemption
